Welcome to the Alive At The Center APS submission site. This is a multi-purpose form, suitable for submission by practicing counselors, counselors in training, or from other interested parties. You can submit your APS here to ensure the fastest turn around time. Be sure you DOUBLE CHECK all of your numbered answers before submitting your APS, that a response number was put in the boxes.

CREATION THERAPY STUDENTS: There is no charge for your taking this questionnaire, as it is pre-paid requirement of your studies. Press submit at the end of the questionnaire. Alive At The Center Ministries will score and return your results directly to you.

PRACTICING COUNSELOR'S AND THERAPISTS: There is a $25.00 charge for processing the APS for you and your counselees. Please be sure that your account with us up-to-date, or submit your payment ASAP after submission.

ALL OTHERS: If you were directed here by Alive At The Center Ministries, Note that there is a $50.00 charge for processing your Personal APS. Your questionnaire will be held for up to 10 days, awaiting your payment. This is the Response Form which the S.A.C.C. uses to collect the needed information to determine your temperament. There are no RIGHT or WRONG answers. Please follow these instructions carefully.

Answer the way that you actually interact with people, NOT how you think you should interact. Do not analyze or debate over your response (i.e. take a lot of time deliberating).
